This form acts a a contract and a purchase agreement. Changes can not be made after submitting this final order form.

Please do not complete this form more than 4 weeks prior to your event date.

We do not send invoices more than 3 weeks prior to your event date.

Please place a separate online order by CLICKING HERE and select pick up at checkout so you are not charged a second delivery fee. In the notes section at checkout, please note that this order is to be added to your existing Catering Package so our team can include and style it within your catering setup.
